It looks like a head and an abdomen with arms coming out just between them.

Hi Pj, I agree with everyone else to get a second opinion but my ultrasound pic looked just like that and I had twins.

could be a head and abdomen or two heads. Regardless, you need to be asking your doctor. If it does turn out to be twins you will need to be vigilant about your prenatal care more than ever. Do yourself and baby/ies a favor and find out by fact and not just asking around.
