Twins or implatation bleed??

A week ago I thought I was miscarrying. I went to the ER and my HCG level was 8,600. They did an ultrasound and saw a gestational sac (no yolk sac or fetal pole) and they also saw an "anterior implantation bleed". They told me that they normally see a fetal pole between 4,000 and 8,000 HCG level and I was possibly miscarrying. Went in on monday for a follow up ultrasound and they saw the gestational sac, yolk sac but no heart rate yet. My HCG level rose to 23,000. I paid for an independent ultrasound today and this was the picture. The rounder sac has a definite fetal pole with heart rate. Looks to me like another gestational sac next to it! Hard to tell what's going on because my uterus is tipped so far back! Anyone have any opinions? PLEASE!!!!!
